Subject Access Request Form (SAR)

You can use this form to request a copy of your medical records held by your GP, The New Surgery, Folkestone, Kent in line with the General Data Protection Regulations (GDPR), Chapter 3, Article 15 (Recitals 63 & 64).

You can also use this form to request a copy the records on behalf of someone else, as long as you are legally allowed to act on their behalf. This includes:

  • Making a request for a child
  • Making a request for someone that you have power of attorney for.

Travel Immunisations Update May 2021

The Government’s website has been updated with details on the vaccination requirement before travelling internationally.

Countries will decide whether they require proof of Covid vaccination for entry, and it is the traveller’s responsibility to check individual requirements. If needed, people in England who have both vaccine doses will be able to demonstrate their Covid vaccination status via the NHS App from 17 May.

Those without access to the app can request a letter from the NHS proving their vaccination status by calling 119.  More information will be shared when it becomes available.

The latest information on symptoms of coronovirus infection and areas where recent travel may have resulted in a high risk of exposure can be found on https://www.nhs.uk/conditions/coronavirus-covid-19/.

NHS 111 has an online coronavirus service that can tell you if you need medical help and advise you what to do.

 Use this service if:

• you think you might have coronavirus

• in the last 14 days you’ve been to a country or area with a high risk of     coronavirus

• you’ve been in close contact with someone with coronavirus.

Do not go to a GP surgery, pharmacy or hospital.

Call 111 if you need to speak to someone.

Everyone is being reminded to follow the public health advice on the NHS website to avoid catching or spreading coronavirus. (Update: 10.03.2020)

We offer the following vaccinations for travel, these are available on the NHS, any additional vaccines would need to be from specialist providers as they are not available on the NHS.  Vaccinations recommended for travel vary according to destination and duration of travel.

Hepatitis A

7th July 2017 – Update:

Dose sparing regimes have been adopted in line with national guidance. Current advice may differ from that previously given as there is a global shortage of adult hepatitis A vaccine.

A full course of vaccination 2 doses, 2nd dose 6-12 months after the 1st,

additional booster may be required 25years after completion of course.

Typhoid

Injection lasts for 3 years.

Oral tablets last for 1 year (not usually given) and cannot be taken with malaria tablets.

Diphtheria, Tetanus and Polio

For adults booster required 10 years after last dose.

Please complete and return a travel risk assessment form, they can be collected from reception or you can print a copy from our documents section. One form is required per patient.

Please be aware that at peak travel times your requests will be dealt with mindful of your travel date. 

Travel Advice Update February 2019

Due to a high demand for appointments currently, please note that requests for travel advice that are submitted at short notice (less than 6 weeks) may struggle to get suitable appointments.  We apologise if this is the case.  Please be aware that travel health advice, travel vaccines and malaria tablets are available in travel clinics and some larger pharmacies.

Health Checks for New Patients

We will ask you to fill in a short health questionnaire. It is useful if you bring your medical card with you and a copy of your previous GP’s repeat prescription slip. We will then write to you to confirm your health check appointment with one of out Health Care Assistants or Practice Nurse. Patients are registered with the practice and not with a GP.

Diabetes, CHD and Asthma

Nurse Sandra offers nursing care to patients who have respiratory diseases, heart disease or diabetes. Please ask at reception regarding appointments. Should you have more than one of these diseases then please ask for a double appointment.

Our diabetic care included retinal screening, podiatry and regular review of blood sugars, blood pressure and other associated problems that may occur within diabetes.

Our care of patients with heart disease has two aims: firstly to prevent the development of heart disease, and secondly to reduce the risk of those who have existing heart disease. This will include blood pressure, cholesterol, blood sugar testing, exercise, diet, smoking cessation, family history, coexisting diseases, medication and symptom review.

Our asthma care helps you to be in control of your asthma and to manage your symptoms. We review your medication and provide help and advice.

Immunisations for Children and Adults

It is important that children are fully immunised. Thanks to immunisation many serious illnesses are now rare, but if caught they can be crippling or even fatal. Please do not forget your children’s pre-school booster and MMR2 vaccination they are just as important to your child and their peers as their baby vaccinations.

If you have any worries about any immunisations please make an appointment to talk to your doctor, your health visitor or the nurse.

Contraceptive Services

The contraceptive pill, the morning after pill and the contraceptive injection are available from doctors and nurses during normal surgery times.

LONG ACTING REVERSIBLE CONTRACEPTIVES  

We offer Depot injections (lasting three months) these are available after discussion with the nurses or doctors. Please feel free to speak to the nurses or the doctors about Long Acting Reversible Contraception.

If you would like to consider this method of contraception please collect a separate leaflet from the waiting room to help you make your decision.

Cervical Smears

Nurses generally take smears at this surgery by appointment. Smear tests are an examination of the neck of the womb (cervix) to check for changes which occur before cancer develops.

These simple tests are recommended for all adult women, with a few exceptions. If you are in doubt, or have worries about this test, please have a word with the nurse or doctor.

Reminder letters are sent out when smears are due.
